In order for a student athlete to be eligible to try-out for a sport at BTHS he/she must have the following:
-Complete Physical dated after June 18, 2011.
-Signed parent permission form.
- Minimum of a cumulative 2.0 GPA.
- Must be less than 19 years 9 months old.
- Must have completed the 8th grade for the first time less than 4 years ago.

TO BE SIGNED AND TURNED IN... Form must be turned in prior to participation in tryouts and dated after June 18th, 2011 (PLEASE USE THIS FORM, NOT THE YELLOW FORM FROM THE HEALTH DEPARTMENT)
Athletes must have a new Parent Consent Form on file before participation in tryouts - Form also provides information regarding the policy Verifies that you have read and understand the Victory with Honor contracts
and the Student-Athlete Code of Conduct contract (all found below)
