BTHS Registration Information<\/span><\/h1>\n

REGISTRATIONS ARE BY APPOINTMENT ONLY<\/span><\/strong><\/p>\n

2026-2027 New and Re-Enrolling\/Transferring Registrations take place during June and July.<\/strong><\/span><\/p>\n

 <\/p>\n

We expect the 2026-2027 New student and Re-Enrolling Student online Registration Application is Now Open!\u00a0<\/u><\/strong><\/span><\/p>\n

 <\/p>\n

We will email you to set up an appointment to complete your students course selection.<\/em><\/strong><\/p>\n

Please make sure documents are legible<\/b> prior to starting the form. Please upload all documents prior to submitting online registration form.<\/b><\/span><\/p>\n

REQUIRED DOCUMENTS TO MOVE TO COURSE SELECTION PORTION OF THE REGISTRATION:<\/strong><\/span><\/p>\n

All students must register at their home-zoned school. The school district\u00a0Attendance Zone Locator<\/a>\u00a0<\/em>will identify the correct school.<\/p>\n

    \n
  1. School Entry Physica<\/strong>l (with Provider Stamp) – dated no more than 1 year prior to the students first day of school.<\/li>\n
  2. Immunizations –<\/strong> on the FL-680 Form. – Out of state immunization must be transcribed onto the<\/strong> FL-680 Form<\/strong><\/span>. This can be done at the Department of Health<\/a> in St. Augustine, some CVS Minute Clinic’s or private -physician. This is State mandated and must be completed for your student to start school. (Religious Exemption also goes through the DOH.)<\/li>\n
  3. Birth Certificate<\/strong>.<\/li>\n
  4. Social Security Card<\/strong> – this is not required but recommended as is needed for Bright Futures, and Colleges and Universities recommend it be on student transcripts.<\/li>\n
  5. Legal Guardianship \/ Custody papers<\/strong> (only if applicable.) ** Please see below for further information.<\/span><\/li>\n
  6. Copy of EP \/ IEP or 504\u00a0<\/strong> (only if applicable.) The lack of this document (if it applies) will slow the registration process.<\/li>\n
  7. Residency<\/strong> (click here for residency information.)<\/a><\/li>\n
  8. Students school records<\/strong> \/ official or unofficial transcripts, report card, transfer grades. Rising Freshman records must show promotion or completion<\/strong>. Parents are required to have Foreign Language Transcripts \/ student records translated, this includes the student Birth Certificate and or passport. <\/strong><\/em><\/span>\u00a0(We will request records; however, we request parents provide records for the course selection meeting – as they are required at the High School Level.)<\/li>\n<\/ol>\n

    **Pursuant to School Board Rule 5.05(1)(e), when a student resides with a person who is not the student\u2019s parent seeks to enroll in school, the student shall present a court order appointing the person with whom they reside as either their legal guardian or legal custodian or shall present other proper documentation from a state or federal agency placing the child with the person with whom they reside.** Required: Written notarized consent from the parent or legal guardian – AND- Documentation from the Clerk of Courts showing that a Concurrent Custody or Temporary Guardianship petition has been filed.\u00a0<\/em><\/span><\/strong><\/p>\n

    Once you have completed the online registration, we<\/strong> will contact you via email regarding Course Selection.\u00a0 The Course selection portion of your students registration will take place via phone.<\/p>\n

    *** We will request records prior to the course selection portion of the registration, however we\u00a0 require that you have at minimum unofficial records. (This is requested\u00a0for BT High School registration.)<\/b><\/span><\/em><\/span><\/p>\n

    All St. Johns County K-8 and high schools are excluded from controlled open enrollment as they are currently operating at full capacity or have been operating for less than three (3) years.<\/u><\/em><\/p>\n
